    • Drop with a “W” grade (with approval) begins: February 3rd
    • Last day to drop with a “W” grade (with approval): April 15th

    This form is only for Art students and can be used from February 3rd to April 15th

IF NO, Please let your instructor know you will be withdrawing from their class.
Withdrawing from a class can have a significant impact if you are a student athlete, international student, or receiving VA education benefits. It can also effect on campus housing, financial aid, and scholarships.
I acknowledge that it is my responsibility to discuss withdrawing from my course(s) with my relevant departments.

Please indicate the following so we may initiate a withdraw on your behalf.

Once you have completed this page, we will initiate the drop process and you will be notified by the Office of the Registrar once the process has been completed.  If this form is submitted after 5 pm, during a holiday or a weekend, the process will not be started until the next business day.

A copy of your responses will be emailed to the address you provided but please understand that you may not see it immediately.  All withdrawals are processed by the Registrar’s Office and will require confirmation from your instructor. If this form is submitted after 5 pm, during a holiday or a weekend, the process will not be started until the next business day.
